David Eric Davis

(Music/Lyrics/Co-Book Writer) A composer, lyricist, visual artist, writer and director, his rock musical comedy F#%KING UP EVERYTHING won Best Book and Honorable Mention for Best Lyrics at NYMF. Davis is the recipient of grants from Theater League and Anna Sosenko Assist Trust and a participant in CAP21’s Writers Co-op. He directed TAROT READING: Love, Sex and Mommy at the NY Fringe Festival and wrote and directed Strike!, a performance piece based on the sounds of bodies moving through air and colliding, at Judson Church. Sam Forman

(Co-Book Writer) Sam Forman is a playwright, lyricist and television writer living in New York City. For the past two seasons, Sam has been part of the writing staff of Netflix’s House of Cards. Sam is also the lyricist for the upcoming musical adaptation of Sleepless In Seattle (premiering spring 2013). Other plays and musicals include: The Rise and Fall of Annie Hall (dir. Jen Wineman

(Director/Choreographer) Recent work includes Donald Margulies’ Shipwrecked! An Entertainment (Triad Stage, Greensboro, NC); Roller Disco The Musical (American Repertory Theater’s OBERON stage, Cambridge, MA), which she co-wrote with Sam Forman and Eli Bolin and also choreographed; Sarah Gancher’s Wake Up (Telluride Theatre, Telluride, CO), and the world premiere of José Rivera’s Lessons for an Unaccustomed Bride (59E59, NYC). Up next for Jen is a new production of Naomi Iizuka’s play Aloha, Say the Pretty Girls for Theatre Vertigo in Portland, OR. Education: B.A. Vassar College, M.F.A. Jeremy is the author of The Bronx Balmers and several short plays. His company, Off The Leash Productions, provides a wide range of video production services. In a previous life, Jeremy was Vice President, Programming for CBS Sports. Jeremy serves on the board of Theater Resources Unlimited.

(General Manager) Lisa Dozier King has general managed fifty productions as well as hundreds of special events in New York City. Current Off Broadway: Sistas: the Musical, Ten Chimneys. Joe Lavezzo (Associate Producer)

(Associate Producer) Joe Lavezzo is new to the world of theatre production, but has always been interested in theatre, taking many classes in acting, directing and producing.  And, as a lifelong resident of New York, he is a frequent and avid member of the audience.  For most of his life, Joe has worked in Information Technology for financial firms.  From the 1970s to the 1990s, Joe was active in New York politics running for the US Congress, New York State Assembly, New York State Senate and Borough President of Manhattan.  Never won. Joe has an MBA from NYU.

CAP 21

(Associate Producer) Currently celebrating its 20th Anniversary, CAP21 is proud to be an FUE team member during this momentous year. CAP21, an award winning organization dedicated to expanding the Theatre Cannon with new plays and musicals, further developed 28 new works last season, including the GLAAD Award-Winning Musical, Southern Comfort. Additionally, for four consecutive years, the writers of new musicals developed and produced at CAP21 have won the Jonathan Larson Grant. CAP21 is home to one of America’s most prestigious musical theatre conservatories.
